Position Summary:

As a member of the multidisciplinary team is responsible for conducting ; assessing physical health needs; making appropriate referrals to community physicians; providing management and nursing delegation of medications in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, training and support services; .

Position Qualifications:

  • Minimum Education: May have a Master's, Bachelor's or Associate degree in nursing.
  • Minimum Field-of-Expertise: Psychiatric nursing; Computer proficiency in various software.
  • Required Certification/Registration: Must be licensed to practice in the state of Hawaii
  • Basic Life Support: (renew upon it's expiration)               
    • CPR   
    • First Aid
    • CPI 
  • Preferred Experience: At least one year of work experience in an inpatient or outpatient psychiatric setting prefer experience working with Developmental Disability participants.

Duties and Responsibilities:

  • Provide daily monitoring for an Assigned group of participants including coordinating and monitoring the activities of the case management team
  • Assume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in conjunction with the DOH case manager and team 
  • Provide individual supportive therapy and symptom management, ensuring immediate changes are made in the treatment plans as participants needs change
  • Educate and support participants' families, and advocate for participants' rights and preferences
  • Conduct comprehensive assessment of psychiatric history, mental status, and diagnosis; physical/dental health; use of drugs or alcohol; education/employment; social development and functioning; activities of daily living; and family structure and relationships
  • Acts as liaison and consult with community agencies and families to maintain coordination in the treatment process
  • Perform shift management  and on-call intervention covering nighttime hours and serve as a backup to evening and weekend staff 
  • Document participant progress to maintain a permanent record of participant activity according to established methods and procedures
  • Participate in daily staff organizational meetings and Positive Behavioral Support/ IP meetings
  • Provide Nursing Delegation and medical services
